Plantar Fasciitis: What the Evidence Says About That First-Step Heel Pain

Plantar fasciitis is the most common cause of heel pain seen in sports medicine and primary care clinics, and it shows up often this time of year as cross country runners build mileage and fall athletes return to two-a-days on hard preseason turf. The hallmark complaint is familiar to almost anyone who's had it: a sharp, bruised-feeling pain in the bottom of the heel with the very first steps out of bed, which eases somewhat with walking but often returns after long periods on your feet.

Here's what current research actually says about why that happens and what the evidence supports for treating it.

It's Less "Inflammation" Than the Name Suggests

The "-itis" in plantar fasciitis implies straightforward inflammation, but tissue studies of chronically painful plantar fascia more often show degenerative changes — disorganized collagen, thickening, and small areas of tissue breakdown — rather than the classic inflammatory picture seen in an acute injury. A 2025 comprehensive review of the diagnosis and management literature describes this shift in understanding as central to how clinicians now approach treatment, since it explains why anti-inflammatory approaches alone are often disappointing for pain that's been present for months.1

Why Mornings Are the Worst

Overnight, the plantar fascia shortens while the foot is unweighted and relatively still. The first steps of the day suddenly stretch that tightened tissue, which is what produces the characteristic sharp morning pain. This mechanical explanation is also why night splints, which hold the foot in a gently stretched position overnight, have a role in some treatment plans.

What the Evidence Actually Supports

Clinical practice guidelines from the Academy of Orthopaedic Physical Therapy, published in the Journal of Orthopaedic & Sports Physical Therapy, recommend plantar-fascia-specific and calf-stretching programs as a first-line approach, supported by manual therapy, foot taping for short-term relief, and night splints for people whose pain is worst with that first morning step.2 Notably, the same guidelines caution against relying on orthotic inserts as a stand-alone treatment — they appear to help more as one piece of a broader plan than as a fix on their own.

For the roughly 90% of cases that improve with these conservative measures over several months, that's usually where treatment stays.1 For pain that doesn't respond to several months of consistent conservative care, a 2026 network meta-analysis of 63 randomized trials compared shockwave therapy, platelet-rich plasma (PRP), corticosteroid injections, and other minimally invasive options, finding that different approaches offer different tradeoffs in short- versus longer-term relief — reinforcing that there's no single "best" procedure and that the choice depends on how long symptoms have persisted and what's already been tried.3

The Takeaway

Most plantar fasciitis is a slow-healing overuse condition, not an emergency, and the evidence favors patience with a structured stretching and loading program before moving toward procedures. Patients in the western suburbs who've been managing heel pain through a full cross country or soccer season without improvement are reasonable candidates for an in-person evaluation to confirm the diagnosis and rule out less common causes of heel pain.
