Platelet-Rich Plasma for Orthopedic Conditions: What Does the Research Actually Say?

Platelet-rich plasma, or PRP, has become one of the more talked-about options in non-operative orthopedic care — and also one of the more inconsistently explained. It's not a single standardized product, and it doesn't work equally well for every condition it's used for. Rather than make a blanket claim in either direction, it's worth walking through what PRP actually is and what the evidence shows, condition by condition.

What PRP Actually Is

PRP is prepared from a patient's own blood: a sample is drawn and spun in a centrifuge to concentrate the platelets, which are then injected into the injured tissue. Activated platelets release a mix of growth factors — including platelet-derived growth factor, transforming growth factor, and vascular endothelial growth factor — that are thought to help stimulate the body's own healing and inflammatory response at the site of injury.1 Because it's autologous (made from the patient's own blood), there's no risk of an allergic or immune reaction to a foreign substance, which is part of why it's generally considered low-risk. What varies enormously from study to study, and from clinic to clinic, is how the PRP is prepared — platelet concentration, whether white blood cells are included, and injection protocol all differ, which is a big reason the research findings aren't always consistent.1

Tennis Elbow (Lateral Epicondylitis): Favorable Long-Term Evidence

This is the condition with the most encouraging data. A 2024 systematic review and meta-analysis of randomized controlled trials comparing PRP to corticosteroid injections found that corticosteroids provided better pain relief in the short term (under two months), but PRP produced significantly better long-term (six months or more) improvement in pain, disability scores, and elbow function.2 In other words, corticosteroids tend to work faster, but PRP's benefit holds up better over time — a pattern that shows up repeatedly in the tennis elbow literature.

Knee Osteoarthritis: Growing, Increasingly Favorable Evidence

The evidence base here has matured considerably. A 2025 systematic review and meta-analysis of randomized controlled trials found that PRP produced meaningfully better pain relief and functional improvement than both saline and corticosteroid injections, with the strongest effect in early-to-moderate osteoarthritis (Kellgren-Lawrence grades 1–3) rather than advanced, bone-on-bone disease.3 Other recent reviews have found PRP outperforming hyaluronic acid (viscosupplementation) as well, particularly when multiple injections are given rather than a single dose.4 As with tennis elbow, preparation method and dosing still aren't fully standardized across studies, which is worth keeping in mind when comparing results.

Rotator Cuff Tendinopathy: Mixed, Still Unsettled

This is the condition where the evidence is least conclusive. Recent systematic reviews and meta-analyses describe short-term pain and functional improvement with PRP for rotator cuff tendinopathy, but inconsistent long-term results, with considerable variation in study design, PRP protocol, and comparison group across the literature.5 One meta-analysis of PRP used as a surgical adjunct during arthroscopic rotator cuff repair did find a lower retear rate with PRP (16.5% versus 23.6%), though other trials in that same space found no difference.5 The honest summary is that PRP shows promise for rotator cuff pathology but hasn't yet built the same level of consistent long-term evidence that exists for tennis elbow or knee osteoarthritis.

The Takeaway: Evidence Varies by Condition

The most useful way to think about PRP isn't "does it work," but "how well does it work for this specific tissue and this specific problem." A professional sports medicine society's 2024 review of the PRP-for-osteoarthritis literature reached a similar conclusion — the evidence has strengthened meaningfully in recent years, but appropriate patient selection and realistic expectations still matter more than the treatment itself.6 That's the framing worth carrying into any conversation about whether PRP is a reasonable option for a particular injury.
