Cross country season is underway across the Chicago suburbs, and with it comes one of running's most common early-season complaints: an aching, diffuse pain along the inner shin that shows up a few weeks into training. Runners and parents often call it "shin splints." In sports medicine, the more precise term is medial tibial stress syndrome, or MTSS — and it's worth understanding both how common it is and what the evidence actually supports for managing it.
How Common Is It?
MTSS is one of the most frequently reported overuse injuries in runners, with reported incidence ranging widely across studies — from roughly 5% up to 35% of runners depending on the population and how the condition was defined.1,2 In high school cross country runners specifically, one commonly cited study found a season incidence of about 12% over an eight-week training block, with certain runners — particularly those newer to the sport — disproportionately affected.3 A separate study of high school runners found the overall injury rate was roughly 2.8 per 1,000 athlete-exposures, with a notably higher rate in female athletes than male athletes.4
What's Actually Happening in the Bone
MTSS produces pain along the posteromedial (inner, lower) border of the tibia, typically in the middle-to-distal third of the shin, and is classically brought on by running or similar repetitive impact and eased by rest.5 It's generally understood as a bone-stress response: repetitive loading outpaces the tibia's ability to remodel and adapt, producing localized bone stress and periosteal irritation. Left unaddressed, this same continuum of bone stress can, in some cases, progress toward a tibial stress fracture — which is part of why early recognition matters.4,5
What Actually Raises the Risk
A systematic review and meta-analysis of risk factors for MTSS in runners identified several consistent contributors: female sex, a prior history of MTSS, use of orthotics, higher body mass index, greater navicular drop (a measure of foot pronation), and reduced ankle plantarflexion range of motion.2,6 Training-related factors matter too — sharp increases in mileage or intensity, running on hard or uneven surfaces, and calf muscle weakness or tightness have also been associated with higher risk.2,6 None of these factors guarantee an athlete will develop MTSS, but they help explain why the same training block affects some runners and not others.
What the Evidence Supports for Recovery
Despite decades of research, there is no single intervention proven clearly superior to relative rest combined with a gradual, graded return to running.1,7 Typical management includes an initial reduction in impact loading, addressing calf flexibility and strength, and a structured, progressive reloading program rather than a hard stop-and-restart; low-risk bone stress injuries like MTSS generally follow a healing timeline in the range of four to eight weeks, though this varies by individual and severity.7,8 Because "run through it" and "rest completely, then resume full mileage" are both associated with higher re-injury risk, a graded plan — and, for a persistent or recurring case, an in-person evaluation with a sports medicine physician — is generally more reliable than guesswork.7,8
