Joint and soft-tissue injections — corticosteroid, PRP, viscosupplementation, and others — have traditionally been performed using surface anatomy alone: the clinician feels for bony landmarks, estimates the joint space, and places the needle without being able to see it. Over the past two decades, portable ultrasound has become common in sports medicine and musculoskeletal clinics, letting the physician watch the needle in real time as it approaches the target. The evidence comparing the two techniques is now substantial, and it's fairly consistent across joints.
Ultrasound Guidance Is Measurably More Accurate
The clearest, most reproducible finding across the literature is accuracy — how often the needle tip actually ends up where it's intended. For hip joint injections, a systematic review and meta-analysis in the British Journal of Sports Medicine found a weighted mean accuracy of 100% for ultrasound-guided injections compared with 72% for landmark-guided injections.1 For knee injections, a 2024 systematic review and meta-analysis of randomized controlled trials found ultrasound guidance achieved 95.8% accuracy versus 77.8% for landmark guidance.2 Shoulder injections show a similar pattern, with image guidance consistently outperforming blind technique across multiple reviews.3
Does Better Accuracy Mean Better Outcomes?
This is where the picture gets more nuanced, and it's worth being direct about it rather than overselling the technology. An umbrella review pooling multiple systematic reviews across joints found that ultrasound guidance reliably improves procedural accuracy and tends to produce better short-term pain and functional outcomes, but the advantage for longer-term effectiveness is less consistent and varies by joint and condition.4 In other words: ultrasound guidance reliably gets the medication where it's supposed to go, and that more often translates into a better result in the weeks after the injection — but a well-performed landmark injection into an accessible joint like the knee can still work well for some patients, and accuracy is only one factor in how an injection ultimately performs.
Why Accuracy Still Matters
Placing an injection outside the intended target isn't just a matter of the medication being less effective — a missed injection means the corticosteroid, PRP, or other agent may end up in surrounding soft tissue rather than the joint or structure it was meant to treat, which can blunt the intended effect and, in some cases, expose nearby tissue to a medication it wasn't meant to receive. Ultrasound also allows the physician to visualize nearby nerves, blood vessels, and tendons in real time, which is particularly relevant for deeper or more anatomically crowded targets like the hip or structures near the shoulder's neurovascular bundle.5
Where This Fits in Sports Medicine
Ultrasound guidance has become a standard tool across orthopedic sports medicine for exactly this reason — it doesn't change what's being injected, but it improves the odds that the injection lands where the physician intended.5,6 It's one of several ways the field has shifted toward real-time imaging and away from technique based purely on surface anatomy, alongside diagnostic musculoskeletal ultrasound used to evaluate tendons, ligaments, and joints during a clinic visit.
